Background: Gout is a form of arthritis characterized by sudden, severe attacks of pain, tenderness, redness, warmth, and swelling in certain joints — usually the large toe, but also the knee, ankle, foot, hand, wrist, and elbow. Gout is caused in part by excessive intake of food rich in uric acid, or decreased excretion of uric acid through the kidneys. As uric acid deposits accumulate in the joints, they cause severe pain.
TCM perspective: Gout is generally considered as bi zheng (painful obstruction syndrome) with damp-heat. The presence of damp-heat begins with the inability of the Spleen and Stomach to adequately transform and transport food, followed by accumulation of damp-heat blocking the channels and collaterals — leading to severe pain. This correlates with the Western concept of gout, which starts with improper diet followed by excessive accumulation of uric acid. Optimal treatment requires herbs to strengthen the Spleen and Stomach, drain damp-heat, open the channels and collaterals, and relieve pain.

DOSAGE
Standard: 4 capsules three times daily. Begin herbal therapy immediately upon noticing the first warning signs of gout. Days 1–2: dosage may be doubled for faster onset of action. Maintenance/prevention: reduced dosage as needed for long-term prevention.
Supplementary formulas: Severe pain — combine with Herbal ANG. Severe swelling and inflammation — combine with Astringent Complex. Gout with obesity — use with Herbalite to reduce body weight. Gout with diabetes — use with Equilibrium to control blood glucose. Gout with hyperlipidemia — use with Cholisma to reduce cholesterol and triglycerides. Gout with obesity and high cholesterol — add Cholisma (ES). Gout with kidney stones — add Dissolve (KS). Hypertension — add Gastrodia Complex or Gentiana Complex.
Nutrition: Hai Zao (Sargassum) and Kun Bu (Ecklonia) are helpful — they contain protein and vital minerals that reduce uric acid levels in the blood. Adequate vitamin C and bioflavonoids (3,000–5,000 mg daily) help reduce uric acid levels in the blood. Essential fatty acids are beneficial for gout — needed to repair tissues and heal joint disorders. Avoid purine-rich foods that increase uric acid: meat, bone broth, gravies, meat extracts, seafood (anchovies, fish roes, herring, sardines, mussels), shellfish, internal organ meats (liver and kidneys), beer and other alcoholic beverages, spinach, asparagus, mushrooms, and cauliflower. Increase intake of cherries, blueberries, and strawberries — excellent for neutralizing uric acid. Always consume adequate distilled water and avoid tap water whenever possible — helps flush out uric acid crystals. After a gout attack, eat only raw fruits and vegetables for approximately 1–2 weeks.
Key principle: Optimal treatment lies not in drugs or herbs alone, but in commitment to diet and lifestyle changes. Long-term success requires decreasing intake and increasing elimination of uric acid.
CASE STUDY
— 63-year-old female (Y.J.): Arthritis and gout pain throughout feet, fingers, wrists, and arms; joint inflammation, redness, inability to extend joints; sharp burning pain; BP 132/61, HR 73; qi and blood stagnation (TCM).
