⚠️ Contraindicated during pregnancy and nursing. Patients with thyrotoxicosis or thyroid storm require emergency medical treatment — warning signs include high fever, tachycardia, fidgeting, irritability, nausea, vomiting, obvious weight loss, profuse perspiration, delirium, and possible loss of consciousness. During the acute inflammatory phase of Grave's disease, Huang Qi (Radix Astragali) should not be used as it may further stimulate the immune system — Gardenia Complex is a more suitable formula during this phase.

CLINICAL APPLICATIONS

* Hyperthyroidism with low-grade fever, tachycardia (90–120 bpm), tremors of the tongue and fingers, enlarged thyroid gland, unilateral or bilateral swollen and bulging eyes

* Hyperthyroidism with palpitations, fatigue, weight loss, fidgeting, irritability, bad temper, aversion to heat, perspiration, hunger and increased appetite, and increased blood pressure

Background: Hyperthyroidism is thyroid hormone excess caused by autoimmune disorder (Grave's disease), iodine excess, or hyperfunctioning (toxic adenoma, toxic multinodular goiter) or inflammation (thyroiditis) of the thyroid glands. Optimal management requires treatment of the cause and balancing of thyroid hormones.

TCM perspective: Hyperthyroidism is a combination of qi and yin deficiencies (the fundamental cause), Liver fire rising, and phlegm stagnation (the symptoms and signs). Treatment must address both the cause and the symptoms simultaneously.

Formula origin: Thyrodex was developed by Professor Xiao-Ping Zhang of Anhui Hospital of Traditional Chinese Medicine. It is an empirical formula designed specifically for hyperthyroidism, used for over 30 years in China with several thousand patients.

Clinical notes: Patients with hyperthyroidism (other than thyrotoxicosis) should notice dramatic improvement within 1–1.5 months. Most symptoms should completely subside within 3–6 months. Protrusion of the eye(s) may persist despite herbal treatment. If using both drugs and herbs, monitor frequently — over-dosage may cause hypothyroid-like signs and symptoms; under-dosage may contribute to hyperthyroid-like signs and symptoms. Suspect hyperthyroidism in: young adults (especially female) with bad temper, hunger with excessive appetite, palpitations, and profuse perspiration; young adults with fidgeting, irritability, early menses with light flow, amenorrhea, and significant weight loss; young adults with sudden onset of difficulty with physical movement, numbness of legs, or possible collapse of leg muscles while walking.

DOSAGE

Standard: 3–4 capsules three times daily. Dosage may need to be adjusted based on age, body weight, severity of condition, and response to treatment. Dramatic improvement expected within 1–1.5 months; most symptoms subside within 3–6 months.

Supplementary formulas: High blood pressure and fast heart rate due to excess fire — add Gardenia Complex. Anxiety and nervousness — add Calm (ES). Insomnia — add Schisandra ZZZ or Calm ZZZ. Hypertension — use Gastrodia Complex or Gentiana Complex. Enlarged thyroid — use with Resolve (AI). Thirst with dry mouth and throat — add Nourish (Fluids). Kidney yin deficiency — add Nourish or Kidney Tonic (Yin). Blood stagnation — add Circulation (SJ). Dysmenorrhea — use Mense-Ease.

Nutrition: Consume plenty of: broccoli, brussel sprouts, cabbage, cauliflower, kale, mustard greens, peaches, pears, soybeans, spinach, and turnips — these foods may help suppress thyroid hormone production. Short-term consumption of iodine-rich foods (sea salt, iodized salt, kelp, sargassum) provides temporary relief via negative feedback — long-term consumption is not recommended as it facilitates thyroid hormone production. Eat a variety of fresh organic fruits and vegetables of all colors. Incorporate high-fiber whole grains and nuts. Drink warm or hot liquids with meals. Foods with antioxidant effects (vitamin A, C, E) are beneficial — citrus fruits, carrots, green leaf vegetables, and green tea. Chew food completely. Always eat breakfast (optimal digestive time: 8–10 a.m. per TCM clock). Allow 2–3 hours between last meal and bedtime. Avoid fast food, processed foods, junk food, artificial sugars, carbonated drinks, meat, greasy food, alcohol, caffeine, tap water, iron supplements, and produce with pesticides. Avoid foods that damage the Spleen: sugar (cake, dessert, candy, chocolate, canned juice, soft drinks, stevia, agave, xylitol, corn syrup), raw or uncooked meats, cooling foods (tofu, tomato, celery, asparagus, bamboo, seaweed, kelp, bitter melon, cucumber, winter melon, watermelon, citrus, banana, papaya, white radish, napa cabbage), white rice and bread, shellfish (crabs, oyster, scallops, clams, lobster, shrimp), fermented foods, dairy products, lamb, beef, goose, duck, and deep-fried or greasy foods. Avoid warm/hot natured foods that damage qi and yin: mango, durian, coffee, alcohol, energy drinks, and spicy/pungent/aromatic vegetables (pepper, garlic, onions, basil, rosemary, cumin, anise, leeks, chives, scallions, thyme, saffron, mustard, chili pepper, wasabi). Avoid artificial coloring and processed meats (nitrites — inflammation and chronic disease).

Tao of Nutrition (Dr. Maoshing Ni and Cathy McNease) — Goiter: Make soup from dried (preferably green) orange peel, carrots, and seaweed. Soak cherry pits in vinegar until they disintegrate, then apply locally.

Lifestyle: Eliminate all stimulants — cigarette smoking, secondhand smoke, and caffeine from tea, coffee, or soft drinks. Avoid vigorous exercise, hot tubs, and saunas. Engage in relaxing exercises such as walking, qi gong, or tai chi chuan. Sleep by 10 p.m. — in TCM, 11 p.m. to 1 a.m. is when yin shifts to yang; the body must be at rest during this time for optimal health.
