⚠️ Contraindicated during pregnancy and nursing. Patients who wear a pacemaker or take antiarrhythmic drugs (Tambocor/flecainide, Procanbid/procainamide) or cardiac glycosides (Lanoxin/digoxin) should not take this formula — Fu Zi (Radix Aconiti Lateralis Praeparata) may interact with these drugs by affecting heart rhythm and potentiating contractile strength. During the acute inflammatory phase of Hashimoto's thyroiditis, Ren Shen (Radix et Rhizoma Ginseng) should be used with caution as it may further stimulate the immune system — Nourish may be a more suitable formula during this phase.

CLINICAL APPLICATIONS

* Hypothyroidism with fatigue, lack of energy, dull facial expression, hoarse voice, drooping eyelids, puffy and swollen eyes and face, weight gain, constipation, aversion to cold, dry hair and skin, low body temperature, decreased heart rate and blood pressure, muscle weakness or sluggishness

* Chronic thyroiditis or Hashimoto's thyroiditis

Background: Hypothyroidism is thyroid hormone deficiency caused by autoimmune disorder (Hashimoto's thyroiditis), iodine deficiency, drugs (lithium, amiodarone, interferon-α), radiation, or hormone imbalance (insufficient TRH or TSH production). It may occur at any age but is most common among the elderly. Optimal management requires treatment of both the cause and the symptoms.

TCM perspective: Hypothyroidism is characterized by yang deficiency. The fundamental etiology is Kidney yang deficiency; complications may involve Kidney yin deficiency and Spleen and Heart yang deficiencies. Treatment must address both yang and yin deficiencies of the organs involved.

Formula basis: Thyro-forte is a contemporary formula based on Jin Gui Shen Qi Wan (Kidney Qi Pill from the Golden Cabinet) by Zhang Zhong-Jing (150–219 CE). The classic formula has six herbs to balance the endocrine system (nourish Liver and Kidney yin) and two herbs to elevate metabolism (tonify Kidney yang). Clinical applications include thyroid disorders, reproductive disorders, diabetes mellitus, and other endocrine conditions. Mechanism of action: stimulates the endocrine system to increase hormone production from the pituitary gland, hypothalamus, and other glands.

Clinical notes: Patients typically notice improvement within one month, starting with increased energy and elevated body temperature. Stabilization and maximum effect may not be observed until 2 months or longer. Monitor progress by checking body temperature, heart rate, and blood pressure first thing in the morning. If using both drugs and herbs, monitor frequently — over-dosage may cause hyperthyroid signs and symptoms; under-dosage may contribute to hypothyroid signs and symptoms. Important: long-term use of thyroid supplements (such as levothyroxine) is associated with loss of up to 13% of bone mass — take calcium supplements to replenish bone mass. Note: patients who have had a complete thyroidectomy cannot produce thyroid hormone and will not benefit from herbs — they must use synthetic thyroid hormone.

DOSAGE

Standard: 4 capsules three times daily on an empty stomach. Dosage may need to be adjusted based on age, body weight, severity of condition, and response to treatment.

Supplementary formulas: Immediate energy boost — add Vibrant. Edema and water accumulation — add Herbal DRX. Tonify Kidney yang — add Kidney Tonic (Yang). Tonify Kidney yin — add Kidney Tonic (Yin). Long-term energy increase — add Imperial Tonic. Constipation — add Gentle Lax (Deficient). Dry hair and skin — add Polygonum 14. Lack of libido — add Vitality. Adrenal insufficiency — use with Adrenal+. Constitutional weakness and deficiency — add Cordyceps 3. Poor appetite or loose stool — add GI Tonic.

Nutrition: Avoid fluoride and chlorine (found in toothpastes and tap water) — they block iodine receptors in the thyroid gland, reducing hormone production; drink steam-distilled water only. Minimize foods that suppress thyroid hormone production: brussel sprouts, peaches, pears, spinach, turnips, cabbage, broccoli, kale, and mustard greens. Encourage vitamin B complex to promote proper energy generation and utilization. Increase iodine-rich foods such as seaweed — iodine is the building block of thyroid hormone. Yang deficiency and cold presentations benefit from increased consumption of meats (especially lamb), lycii, longan fruit, and warming spices: pepper, garlic, onions, basil, rosemary, cumin, anise, leeks, chives, scallions, thyme, saffron, mustard, chili pepper, and wasabi. Avoid cooling foods: tofu, tomato, celery, asparagus, bamboo, seaweed, kelp, bitter melon, cucumber, winter melon, oranges, grapefruit, pear, banana, watermelon, white radish, napa cabbage, and bamboo sprout — add 20 pieces of Gou Qi Zi (Fructus Lycii) when cooking to make the property more neutral.

Tao of Nutrition (Dr. Maoshing Ni and Cathy McNease) — Goiter: Make soup from dried (preferably green) orange peel, carrots, and seaweed. Soak cherry pits in vinegar until they disintegrate, then apply locally.

Lifestyle: Regular exercise stimulates thyroid hormone secretion, increases tissue sensitivity to the hormone, and raises basal body metabolism. Induce perspiration by taking hot water baths, saunas, or steam baths.
