Neck & Shoulder (CR)™

TCM herbal formula for chronic neck and shoulder pain — numbness, stiffness, discomfort, limited mobility, slow recovery or continuing deterioration, repetitive stress syndrome (prolonged sitting, computer work), over-exertion injuries, arthritis of the neck, and degenerative disc disease — chondroprotective; osteogenic; anti-inflammatory; analgesic; antispasmodic; muscle-relaxant; disperses painful obstruction; strengthens sinews and tendons; disperses residual qi and blood stagnation in channels and collaterals. Formulated by Dr. Alex Chen (30+ years clinical experience); herbs routinely used in trauma departments of hospitals in China.

⚠️ Do NOT take during acute phases of injuries where there is still bleeding, inflammation, or bruising — use Neck & Shoulder (AC) instead. Contains Dang Gui Wei (Extremitas Radix Angelicae Sinensis) — patients on anticoagulant or antiplatelet therapies such as Coumadin (warfarin) should use with caution or not at all (higher risk of bleeding and bruising). If overall condition does not improve after 2–3 weeks, check for structural and anatomical abnormalities.

CLINICAL APPLICATIONS

* Chronic neck and shoulder pain, including numbness and discomfort

* Limited movement of the neck and shoulders due to pain and stiffness

* Long-term neck and shoulder injuries with slow recovery or continuing deterioration

* Injuries of the neck and shoulder muscles commonly caused by over-exertion

* Repetitive stress syndrome of the neck and shoulders (prolonged upright sitting, computer work)

* Arthritis of the neck; degenerative disc disease

Background: Musculoskeletal and connective tissue injuries lead to over 10 million clinic visits per year in the United States. Chronic injuries are characterized by dull pain, stiffness and numbness, and decreased muscle mass and strength — the chronic nature of these conditions may eventually contribute to atrophy and degeneration of soft tissues, decreased mobility of the joints, and generalized weakness and pain. Effective treatment must focus on activating qi and blood circulation, opening the channels and collaterals, and nourishing the muscles and tendons. TCM pain management targets both the symptoms and causes of pain, and is often superior to drugs for chronic pain management — better effects with fewer side effects.

Key herb-level clinical data:

Yan Hu Suo (Rhizoma Corydalis / Corydalis Rhizome): one of the strongest and most potent herbs for treatment of pain; analgesic effect comparable to morphine — much safer, significantly fewer side effects, slower development of tolerance, no evidence of physical dependence; used with satisfactory anesthetic effect in 98 of 105 patients (93.4%) who underwent surgery; analgesic effect potentiated further with concurrent electro-acupuncture; invigorates blood; activates qi; alleviates pain

Ru Xiang (Gummi Olibanum / Frankincense / Boswellia): analgesic; anti-inflammatory — inhibits nitric oxide production; antiarthritic — reduces edema and decreases arthritic scores in adjuvant-induced arthritis; suppresses pro-inflammatory cytokines TNF-α and IL-1β; clinically used to treat pain associated with various types of trauma and external injuries

Mo Yao (Myrrha / Myrrh): analgesic; anti-inflammatory; antiarthritic — reduces edema and decreases arthritic scores in adjuvant-induced arthritis; suppresses TNF-α and IL-1β; clinically used to treat pain associated with trauma and external injuries

Chuan Niu Xi (Radix Cyathulae / Cyathula Root): chondroprotective — potently inhibits MMP-13 (key enzyme for degradation of cartilage collagen matrix, especially under arthritic conditions); protects cartilage from repetitive and stress-induced injuries; excellent herb for cartilage protection

Dang Gui Wei (Extremitas Radix Angelicae Sinensis / Angelica Sinensis Tail / Dong Quai Tail): osteogenic — stimulates proliferation, alkaline phosphatase activity, protein secretion, and type I collagen synthesis of human osteoprecursor cells in a dose-dependent manner; analgesic — approximately 1.7× stronger than acetylsalicylic acid; anti-inflammatory — approximately 1.1× stronger than acetylsalicylic acid; inhibits nitric oxide and prostaglandin E2 in peritoneal macrophages; invigorates blood; removes residual stasis; caution with anticoagulants

Sang Ji Sheng (Herba Taxilli / Loranthus / Mulberry Mistletoe): osteogenic and antiosteoporotic — increases proliferation and differentiation of osteoblasts without affecting osteoclast activity; effectively increases rate of tissue regeneration of damaged bones; tonifies Kidney and Liver; strengthens tendons; alleviates pain, stiffness, and soreness

Xu Duan (Radix Dipsaci / Teasel Root / Dipsacus): osteogenic and antiosteoporotic — promotes generation of new bones; tonifies Kidney and Liver; strengthens tendons; alleviates pain, stiffness, and soreness

Di Huang (Radix Rehmanniae / Rehmannia): anti-inflammatory — increases plasma levels of adrenocortical hormones via the endocrine system; effective even in the presence of dexamethasone; clinical study: used in herbal formula to successfully treat 12 patients with rheumatoid arthritis; tonifies yin; clears deficiency heat

Gan Cao (Radix et Rhizoma Glycyrrhizae / Licorice Root): marked anti-inflammatory — enhances glucocorticoid effect; glycyrrhizin and glycyrrhetinic acid — anti-inflammatory potency approximately 1:10 vs. cortisone; muscle-relaxant — relaxes smooth and skeletal muscles; clinical applications: pain, inflammation, edema, arthritis, spasms, cramps; harmonizes the formula

Bai Shao (Radix Paeoniae Alba / White Peony Root): antispasmodic — relaxes smooth and skeletal muscles; combined with Gan Cao (Shao Yao Gan Cao Tang): marked analgesic and muscle-relaxant effect — potentiated with acupuncture; clinical applications: leg cramps, hemodialysis cramps, restless leg syndrome, intestinal spasm, facial spasms, menstrual cramps, neck pain, sciatica, neuralgia, trigeminal neuralgia, back pain, heel pain, dysmenorrhea

Ge Gen (Radix Puerariae Lobatae / Kudzu Root / Pueraria): isoflavonoid compounds — significant analgesic and muscle-relaxant activities; guiding herb that enhances the effect of all herbs in the formula

Chuan Xiong (Rhizoma Chuanxiong / Ligusticum / Szechuan Lovage Root): anti-inflammatory — inhibits TNF-α production and bioactivity via phthalide lactones (NF-κB pathway); invigorates blood; removes residual stasis in channels and collaterals

Qiang Huo (Rhizoma et Radix Notopterygii / Notopterygium): analgesic — relieves soreness, pain, and numbness in neck, upper back, and shoulders due to wind-damp obstruction; anti-inflammatory — inhibits 5-lipoxygenase and cyclooxygenase

Qin Jiao (Radix Gentianae Macrophyllae / Large-Leaf Gentian Root / Macrophylla): dispels painful obstruction and cramping; relaxes sinews; unblocks channels

Mu Gua (Fructus Chaenomelis / Chinese Quince / Chaenomeles): dispels painful obstruction and cramping; relaxes sinews; unblocks channels

Wu Jia Pi (Cortex Acanthopanacis / Acanthopanax Bark / Siberian Ginseng Bark): treats painful obstruction due to Liver and Kidney yin deficiencies

Tao Ren (Semen Persicae / Peach Seed): invigorates blood; removes residual stasis in channels and collaterals

Hong Hua (Flos Carthami / Safflower / Carthamus): invigorates blood; removes residual stasis in channels and collaterals

Di Gu Pi (Cortex Lycii / Lycium Root Bark / Wolfberry Root Bark): tonifies yin; clears deficiency heat; keeps the temperature of this formula cool

DOSAGE

Standard: 3–4 capsules three times daily on an empty stomach with warm water. May increase to 8 capsules three times daily for 3–4 days to relieve pain. Do NOT take during acute phases of injuries (bleeding, inflammation, bruising).

Supplementary formulas: Bone spurs or calcification — add Flex (SPR). Joint pain or arthritis worsening in cold and rainy weather — add Flex (CD). Joint inflammation with redness, swelling, and burning pain — add Flex (Heat). Severe spasms — add Flex (SC). Strengthen soft tissues (muscles, tendons, ligaments) — add Flex (MLT). Bone fractures, whiplash, or acute injuries — add Flex (TMX). Improve blood circulation — add Flex (NP). Lower back, sciatic, or nerve pain — add Back Support (AC) or Back Support (CR). Acute headache — add Corydalin (AC). Chronic headache — add Corydalin (CR). Osteoporosis — add Osteo 8. Enhance pain relief — add Herbal ANG. Herniated disk — add Back Support (HD). Old injury or stubborn pain due to chronic blood stagnation — add Circulation (SJ). Shooting pain radiating down the arms — add Arm Support. Stress-related neck pain with insomnia — add Calm ZZZ.

Nutrition: Eat plenty of whole grains, dark green vegetables, and nuts — rich in vitamin B complex and magnesium, essential for nerve health and relaxation of tense muscles. Ensure adequate intake of calcium and potassium — deficiency leads to spasms, cramps, and tense muscles.

Lifestyle: Avoid exposing affected areas to cold temperatures or drafts — wear turtlenecks or adequate clothing to cover the neck and shoulder areas. Patients with frozen shoulder should exercise the shoulders as much as possible — increasing range of motion helps prevent adhesions of tendons and ligaments. Check pillow height — not too high or too low. Assess mattress firmness. Hot baths with Epsom salts help relax tense muscles and withdraw toxins from tissues — rest and relax for about 30 minutes.
