Herbal DRX™

TCM herbal diuretic formula for edema, generalized water accumulation, swelling, and tan yin (phlegm retention) syndrome — diuretic; strengthens Spleen; resolves dampness; promotes urination; cardiovascular; hepatoprotective; nephroprotective

⚠️ Contraindicated during pregnancy. Do not use concurrently with diuretic drugs (Lasix, Aldactone, Dyazide) — concurrent use may cause dehydration and electrolyte imbalance. If taken for more than 2 weeks, supplement electrolytes to prevent imbalance — symptoms include muscle spasms and cramps; common supplements include bananas, orange juice, multivitamins and minerals, and sports drinks. Diuretic herbs have moderate potency and slower onset than drugs — most patients notice noticeable diuretic effect only after 2–4 weeks. Acute conditions such as acute nephritis with edema should be treated with other methods.

Background: Edema is the swelling of soft tissues due to increased accumulation of water and fluids. It may be localized (venous obstruction, lymphatic obstruction, or infection) or generalized (heart failure, liver failure, or kidney failure). Additional symptoms include feeling of tightness or fullness in the chest, heaviness or sluggishness in the body, and preference to lay down or sleep all day. In Western medicine, edema is treated with diuretic drugs such as Lasix (furosemide), Aldactone (spironolactone), and Dyazide (HCTZ/triamterene) — all effective but with risks including dizziness, vertigo, orthostatic hypotension, electrolyte imbalance, and in severe cases, agranulocytosis, leukopenia, and thrombopenia. Long-term use may increase plasma glucose and cholesterol, complicating cardiovascular management.

TCM differentiation: Water accumulation is treated with herbs that address both symptoms and causes. Diuretic herbs (herbs that regulate water and resolve dampness) facilitate elimination of water through urination. Additional herbs strengthen the body and increase its own ability to regulate water circulation. Herbal DRX treats the cause by strengthening the Spleen and resolving dampness, and treats the symptoms by clearing damp-heat and draining water through urination.

Severity check: Press the patient's shin bone (around the area of Ligou / LR 5). The deeper the indentation and the longer it stays without bouncing back, the more severe the condition.

Fu Ling (Poria): marked diuretic effect — significant increase in urine output; clinical study: formula containing Fu Ling effectively treated 23 of 30 patients with edema; strengthens Spleen; resolves dampness

Zhu Ling (Polyporus): marked diuretic effect — increases excretion of water, sodium, chloride, and potassium; clinical study: Zhu Ling and Lasix (furosemide) used for 8 days showed similar effect on diuresis, natriuresis, and chloriuresis (AQP1, AQP2, AQP3, V2R expression in kidney medulla)

DOSAGE

Standard: 3–4 capsules three times daily on an empty stomach for maximum effectiveness. Long-term therapy — most patients notice diuretic effect after 2–4 weeks. Supplement electrolytes if used for more than 2 weeks.

Supplementary formulas: Excessive damp and phlegm — add Pinellia Complex. Deficient-type hypertension with edema — add Gastrodia Complex. Excess-type hypertension with edema — add Gentiana Complex. Nutrition: Low-sodium diet — sodium causes fluid retention. Adequate vitamin B complex — helps reduce water retention. Sufficient free-form amino acid complex — edema is sometimes caused by inadequate protein assimilation. Increase foods with natural diuretic effects: Lu Dou (Semen Phaseoli Radiati / Mung Bean) and Yi Yi Ren (Coix / Job's Tears). Avoid ice, cold beverages, and cold or raw foods. Avoid citrus and cold-natured foods: watermelon, salads, tomatoes, and cucumbers. Avoid fried, greasy, or high-fat foods. Reduce dairy and sugar intake.

Tao of Nutrition — Edema and Swelling (Dr. Maoshing Ni and Cathy McNease): Eat red (azuki) beans, corn, ginger skin, winter melon, winter melon skin, squash, apples, mulberries, peaches, tangerines, coconuts, seaweeds, fish, celery, green onions, garlic, bamboo shoots, spinach, water chestnuts, millet, wheat, black beans, pearl barley, carrots, watermelon, oats, and beef. Avoid rich foods, salty foods, lamb, stimulating foods, wine, pepper, shellfish, fatty foods, and greasy foods.

Lifestyle: Sit with legs elevated as much as possible — wearing support hose also reduces leg swelling. Exercise daily to push fluids away from the legs and lower body. Avoid exposure to rain; do not engage in water sport activities during treatment.

CASE STUDY

S.S., 31-year-old male: Desire to lose weight (20 lbs); generalized weight gain; considerable swelling on face and limbs; pitting edema on lower legs and hands; Spleen qi deficiency with dampness and food accumulation.
