TCM herbal formula for hot arthritis, gout, rheumatoid arthritis, and fibromyalgia with inflammation — analgesic; anti-inflammatory; antirheumatic; immunomodulatory; immunosuppressive; chondroprotective; osteogenic; expels wind and heat; clears damp-heat; disperses painful obstruction

⚠️ Contraindicated during pregnancy and nursing. Contains Dang Gui Wei (Extremitas Radix Angelicae Sinensis), which invigorates blood circulation — patients on anticoagulant or antiplatelet therapies such as Coumadin (warfarin) should use with caution or not at all due to higher risk of bleeding and bruising. Do not use if arthritis worsens with cold and rainy weather — use Flex (CD) instead. Do not use heat packs for joint pain due to heat. Rule out internal hemorrhage before treating patients with head injuries.

Background: Musculoskeletal and connective tissue disorders are major causes of pain and physical disability. Simple causes include trauma and external injuries (pulled muscles, strained ligaments, dislocated joints, bone fractures); complicated causes include infection, autoimmune disorders, crystal-induced inflammation, and non-inflammatory tissue degeneration. Optimal treatment must address the symptoms (pain and inflammation) and the underlying causes. After the disorder is stabilized, physical therapy and exercise are important to maintain flexibility and strengthen surrounding muscles.

Flex (Heat) vs. Flex (CD): Flex (Heat) is for arthritis and joint pain with heat signs — redness, burning, swelling, and inflammation. Flex (CD) is for arthritis and joint pain that worsens with cold and damp weather. Use ice packs (not heat packs) with Flex (Heat). Use heat pads (not ice packs) with Flex (CD).

Based on: Er Miao San (Two-Marvel Powder) — the classical formula for damp-heat in the lower body (Cang Zhu + Huang Bo).

Cang Zhu (Rhizoma Atractylodis / Atractylodes): dries dampness; combined with Huang Bo as Er Miao San — excellent effect to treat pain in the lower half of the body

Huang Bo (Cortex Phellodendri Chinensis / Phellodendron): clears damp-heat in the lower body; anti-inflammatory — inhibits IL-1β and TNF-α production in a dose-dependent manner (NF-κB pathway); gout — reduces serum uric acid levels and inhibits liver xanthine oxidase activity

DOSAGE

Standard: 3–4 capsules three times daily on an empty stomach with warm water. Severe pain: 5–6 capsules every 4–6 hours as needed.

Clinical notes: Can be combined with Flex (TMX) in the early stages of sports injuries when there is severe pain with inflammation, swelling, and redness — begin treating immediately for maximum effectiveness. Use ice packs (not heat packs) to reduce redness, swelling, and inflammation.

Supplementary formulas: Enhanced analgesia — add Herbal ANG. Severe inflammation — combine with Astringent Complex. Excess heat — add Gardenia Complex. Severe pain with blood stagnation — add Circulation (SJ). Gout — use Flex (GT) instead. Neck and shoulder pain — add Neck & Shoulder (AC) or Neck & Shoulder (CR). Lower back pain — add Back Support (AC) or Back Support (CR). Herniated disk with swelling and inflammation — add Back Support (HD). Knee pain — add Knee & Ankle (AC) or Knee & Ankle (CR). Nutrition: Sulfur helps calcium absorption — essential for repair and rebuilding of bones, tendons, cartilage, and connective tissues; eat foods high in sulfur such as asparagus, eggs, fresh garlic, and onions. Histidine (an amino acid) removes excess copper and iron found in arthritic patients — eat foods high in histidine such as rice, wheat, and rye. Fresh pineapple contains bromelain — excellent for reducing inflammation. Increase intake of nourishing, cooling foods such as Mexican yam, yam, radishes, potatoes, carrots, melons, cucumbers, beets, turnips, malanga, celeriac, taro, and rutabaga. Avoid spicy food, caffeine, citrus fruits, sugar, milk, dairy products, and red meat. Fish oil may help alleviate pain associated with rheumatoid arthritis — can be taken alongside the herbs. Decrease sour foods, drinks, and citrus — their constricting nature may contribute to further stagnation in the channels and collaterals. Avoid spicy, pungent, and aromatic vegetables such as pepper, garlic, onions, basil, rosemary, cumin, fennel, anise, leeks, chives, scallions, thyme, mustard, chili pepper, and wasabi.

Tao of Nutrition (Dr. Maoshing Ni and Cathy McNease): Recommended foods: plenty of fresh fruits and vegetables, dandelion, cabbage, mung beans, winter melon, and soybean sprouts. Apply poultices of crushed dandelion greens, changing every two hours. Avoid spicy foods, green onions, alcohol, smoking, and all types of stress.

Lifestyle: For overweight patients, weight loss is suggested — lessens pressure on joints and helps relieve pain.

CASE STUDIES

85-year-old female: Severe right toe pain with difficulty walking; BP 140/90, HR 76; damp-heat arthritis (TCM); uric acid buildup (Western). Flex (Heat) 4 capsules 3x/day for 2 weeks + Zheng Gu Shui topically to big toe + distilled water + pure cherry juice + no alcohol. Result: improvement after a few days; completed full course.

54-year-old female (E.K.): Severe burning sacral pain; MRI showed bulging lumbar disc; first time experiencing burning sensation on top of chronic low back pain; heat in lower jiao (TCM).
