Back Support (CR)™

TCM herbal formula for chronic low back pain, sciatica, and osteoarthritis — chondroprotective; osteogenic; anti-inflammatory; analgesic; antispasmodic; disperses painful obstruction; invigorates qi and blood circulation; tonifies Liver and Kidney deficiencies; strengthens bones, sinews, and muscles

⚠️ Not recommended during pregnancy or nursing due to blood-invigorating nature. This is a warm formula — blood pressure should be monitored while taking it as use may be associated with a slight increase. Do not take during the acute phase of injury where there is still bleeding, inflammation, or bruising — use Back Support (AC) instead. Patients with pain radiating to the extremities with sudden loss of bladder or bowel control may have cauda equina syndrome — refer to emergency care immediately as this can lead to permanent disability. If fever accompanies one-sided back pain, consider kidney infection and do not use — refer to medical doctor immediately.

CLINICAL APPLICATIONS

* Chronic low back pain (lumbago)

* Weakness and soreness of the lower back and knees

* Dull achy back pain, stiffness, and limited range of motion

* Slow and incomplete recovery from back injuries

* Sciatica, osteoarthritis, lumbago, lower back pain resulting from osteoporosis

Background: Musculoskeletal and connective tissue injuries lead to over 10 million clinic visits per year in the United States. Causes may be external (sports injuries, car accidents, trauma), internal (chronic wear and tear of muscles, ligaments, and tendons; bones weakened by osteoporosis), or both. Chronic injuries are characterized by dull pain, stiffness, numbness, and decreased muscle mass and strength — treatment includes relief of pain and restoration of physical and physiological functions. Back Support (CR) is formulated based on Du Huo Ji Sheng Tang (Angelica Pubescens and Taxillus Decoction), the classical herbal formula used to treat chronic low back pain, sciatica, rheumatoid arthritis, and osteoarthritis.

Back Support (AC) vs. Back Support (CR): Back Support (AC) is for acute cases with inflammation and excruciating pain — best for recent injury or acute exacerbation. Back Support (CR) is for chronic or dull pain — in addition to relieving pain, it also nourishes and strengthens the underlying structures and tissues so the back becomes less fragile over time.

Key herb-level clinical data:

Du Huo (Radix Angelicae Pubescentis / Angelica Pubescens): large dose — dispels wind, cold, and dampness in the back and lower body; mild sedative; marked analgesic and anti-inflammatory effects

Du Zhong (Cortex Eucommiae / Eucommia Bark): replenishes Liver and Kidney — strengthens bones, sinews, and muscles of the lower back and knees; osteogenic and antiosteoporotic — promotes osteoblast proliferation and differentiation, improves bone mineral density and trabecular microarchitecture; 16-week study: protects against estrogen deficiency-induced osteoporosis without hyperplastic effects on the uterus — potential alternative for postmenopausal osteoporosis; anti-inflammatory — stimulates endocrine system and adrenal cortex steroid secretion; clinically: sciatica (6 patients in herbal formula)

Sang Ji Sheng (Herba Taxilli / Taxillus / Loranthus): replenishes Liver and Kidney — strengthens bones, sinews, and muscles; osteogenic and antiosteoporotic — promotes osteoblast proliferation and differentiation; increases rate of tissue regeneration of damaged bones

Di Huang (Radix Rehmanniae / Rehmannia): replenishes Liver and Kidney — strengthens bones, sinews, and muscles; anti-inflammatory — reduces swelling and inflammation via influence on the endocrine system; clinically: rheumatoid arthritis (12 patients in herbal formula)

Bai Shao (Radix Paeoniae Alba / White Peony): analgesic, antispasmodic, and anti-inflammatory; nourishes and relaxes tendons and muscles of the back; modulates pro-inflammatory mediators from macrophage-like synoviocytes — treats arthritis

Zhi Gan Cao (Radix et Rhizoma Glycyrrhizae Praeparata cum Melle / Honey-Prepared Licorice): harmonizes formula; nourishes and relaxes tendons and muscles of the back; anti-inflammatory — enhances glucocorticoid effect (increased production and secretion, decreased liver metabolism, increased plasma concentration)

Bai Shao + Zhi Gan Cao combined: marked analgesic, anti-inflammatory, and muscle-relaxant effects; clinically: 84.67% effective (105 of 124 subjects) for general pain; lower back and leg pain in 33 elderly patients; severe back and leg pain in 27 patients; neuralgia in 30 of 42 patients; facial spasms in 11 patients; intestinal spasms in 254 patients; intestinal cramps in 85 patients

Mu Gua (Fructus Chaenomelis / Chinese Quince / Chaenomeles): increases qi and blood circulation; relaxes muscles and tendons; anti-inflammatory — prevents ultrastructural changes of synoviocytes; inhibits IL-1, TNF-α, and PGE2; treats rheumatic and rheumatoid arthritis

Wei Ling Xian (Radix et Rhizoma Clematidis / Clematis): increases qi and blood circulation; relaxes muscles and tendons; anti-inflammatory — blocks nitric oxide and prostaglandin E(2); chondroprotective — inhibits MMP-3, MMP-13, and COX-2; prevents extracellular matrix degradation and chondrocyte injury in osteoarthritis; prevents apoptosis in articular chondrocytes (TNF-TRAIL); inhibits pro-inflammatory compounds and prevents rheumatoid arthritis

DOSAGE

Standard: 3–4 capsules three times daily. Maintenance/prevention: 2 capsules daily to strengthen muscles and tendons and prevent injury — recommended for patients with chronic back pain from repetitive occupational movements. Take on an empty stomach with warm water. Do not take during the acute phase of injury where there is still bleeding, inflammation, or bruising.

Supplementary formulas: Weakness and soreness of knees — add Knee & Ankle (CR). Weakened soft tissues (muscles, tendons, ligaments) — add Flex (MLT). Enhanced analgesia — add Herbal ANG. Osteoporosis or bone weakness — add Osteo 8. Herniated disk — use with Back Support (HD). Kidney yang deficiency with low sex drive — add Kidney Tonic (Yang). Kidney yin deficiency — add Kidney Tonic (Yin). Bone fractures and internal bruises — add Flex (TMX). Cold/rainy weather arthritis — add Flex (CD). Hot arthritis with redness and burning pain — add Flex (Heat). Muscle spasms and cramps — add Flex (SC). Bone spurs — add Flex (SPR). Kidney stone back pain — use Dissolve (KS). Nephritis back pain — use Kidney DTX. Menstrual dull back pain — add Mense-Ease. Obesity-related back pain — use Herbalite. Chronic stubborn back pain with blood stagnation — add Circulation (SJ). Inflammation — add Astringent Complex.

Nutrition: Eat a wide variety of raw vegetables, fruits, and whole grain cereals for complete nutrients for bones, nerves, and muscles. Adequate calcium is essential for repair and rebuilding of bones, tendons, cartilage, and connective tissues. Fresh pineapple contains bromelain — excellent for reducing inflammation. To relieve cramps and spasms, eat foods high in potassium such as bananas and oranges, and drink adequate water — back pain may be partly due to dehydration. Deficiency of calcium and potassium leads to spasms, cramps, and tense muscles. Avoid red meat and seafood — high uric acid adds strain on the kidneys. Avoid cold beverages, ice cream, caffeine, sugar, tomatoes, milk, and dairy products.

Tao of Nutrition (Dr. Maoshing Ni and Cathy McNease) — to tonify the Kidney: Make soup with green beans, black beans, azuki beans, and a pinch of cinnamon. Slowly cook ½ cup black beans, ½ cup water, and ¾ cup rice wine for 2–3 hours. Eat a handful of curry cashews daily (dry roast cashews in a pan, sprinkle with curry powder on high flame for a couple of minutes — remove promptly and avoid burning). Eat a small handful daily of pecans soaked in sherry or port for one week or more.

Lifestyle: Use your legs (not your back) when lifting heavy objects. Stretching and strengthening exercises for back and abdominal muscles are essential for long-term recovery. Mild regular exercise — swimming, yoga, or tai chi chuan — is recommended. Weight loss is suggested to decrease pressure on joints and relieve pain. While sitting, keep back straight with elbows and knees at 90° — take a break at least once every hour to alleviate pressure on vertebrae and disks. Mild back pain can be relieved with heat to stimulate blood circulation — massage, hot packs, saunas, and whirlpools are recommended. Hot baths with Ai Ye (Folium Artemisiae Argyi) or Epsom salts (15–30 minutes) relax tense muscles, invigorate blood flow, and draw toxins from tissues — mix 2–3 tablespoons of Ai Ye extract powder in hot water each time. Wear clothing that covers the back completely and tuck shirts in to avoid wind exposure — wind aggravates the condition. Firm beds are recommended over soft ones. Adequate rest is essential to recovery.
